Progressive Benefit Solutions, LLC collects nonpublic personal information about you from the following sources:
Information we receive from your employer and from your enrollment or other forms, such as your name, social security number or other unique identifier, and the amount of your annual plan election; and Information about your transactions with us or others, such as your account balance, and the date, time, amount, payees and outstanding balances related to your transactions.
We may disclose nonpublic personal information about you, such as your name, social security number or other unique identifier, and your plan effective and termination dates, to third party service providers (such as benefit administrators that determine pharmacy benefits under group plans) that are not processing transactions for your plan. We may also disclose nonpublic personal information about you to nonaffiliated third parties as permitted by law.
Should you terminate your participation in the plan for any reason, we will not disclose nonpublic information about you to anyone, except as permitted by law. We restrict access to your personal and account information to those employees who need to know that information to provide products or services to you. We maintain physical, electronic and procedural safeguards that comply with federal regulations to guard your nonpublic personal information.
If you prefer that we not disclose nonpublic personal information about you to nonaffiliated third parties, you may opt out of those disclosures; that is, you may direct us not to make those disclosures (other than disclosures permitted by law). If you wish to opt out of disclosures to nonaffiliated third parties, you may call the number on the back of your Benny Card. If you opt out of these disclosures you will not longer be able to use your Benny Card.
